Swasthya Kavach Family Health Insurance Policy

• 18 years to 60 years of age of the proposer.
• Residents of India from any nationality.
• Coverage available for proposer and up to 3 dependents (children, siblings, nephews or nieces between 3 months to 23 years of age).

Two plans available – Base Plan and Wider Plan.

• Domiciliary hospitalization expenses up to a maximum of 20% of the basic sum insured.
• Hospitalization expenses of persons donating organs during organ transplant of the insured.
• Prescribed package charges of hospitals for specific treatments up to 80% of sum insured or 80% of actual expenses, whichever is less.
• Nursing expenses including those which occur during pre and post hospitalization periods.
• All types of medical practitioner’s fees.
• Cost of blood, medicine, oxygen, anesthesia, diagnostic and pathologic tests, operation theatre charges, dialysis, prosthetic limbs, etc.
• Registration, service charges and other hospital charges subject to a maximum of 0.5% of the sum insured.

• No medical check-up required up to the age of 45 years.
• Free enhancement of sum insured (cumulative bonus) of 5% is granted per claim free year, upon renewal, up to a maximum of 50%.
• Premium depends on the sum insured and the plan opted for, number of family members and age of the eldest insured person.
• Optional critical illness extension can be taken on the Wider Plan – this pays out double the sum insured on diagnosis on 10 predetermined critical illnesses, for an extra 30% on premium.
• Special discount for IFFCO Tokio Motor Two Wheeler policyholders. Discount also available if the insured person is already insured under an IFFCO Tokio group policy.
• No “excess” or “co-op pay” under the policy and the full eligible claim is paid without any deductions (except if mentioned under exclusions).

• All medical conditions pre-existing at the time of policy issuance.
• All diseases contracted within the first 30 days from the policy commencement date.
• Intentional self-injury or attempted suicide.
• Cosmetic or aesthetic treatment.
• Alcohol or drug abuse.
• HIV / AIDS, STDs, Venereal diseases, congenital diseases.
• Cost of spectacles, contact lenses or hearing aids.
• Dental treatment not requiring hospitalization.
• Pregnancy or childbirth.
• Dangerous and hazardous sports and activities.
• Genetic disorders, stem cell implantation or surgery.
• Alternative medicine therapies like naturopathy, acupuncture, magnetic therapy, etc.
• First 10% of any admissible claim, if an insured person is suffering from either diabetes or hypertension and first 25% of claim, if he is suffering from both diabetes and hypertension. This provision is applicable for claims arising out of diabetes and/or hypertension only.
